Diaphragmatic Flutter (Diaphragm Flutter)Diafragma-flutter (diaphragmatic flutter)

A rare diaphragm condition that is often mistaken for hyperventilation Een zeldzame aandoening van het middenrif die vaak voor hyperventilatie wordt aangezien

Anatomical illustration of the diaphragm below the rib cage – the muscle affected by diaphragmatic flutter

Diaphragmatic flutter (also known as diaphragm flutter, diaphragmatic myoclonus, belly dancer’s dyskinesia or Leeuwenhoek’s disease) is a rare condition in which the diaphragm contracts involuntarily and rhythmically. This can cause visible abdominal wall movements, breathing difficulties, pain and a significantly reduced quality of life. Because the complaints resemble hyperventilation, the diagnosis is often delayed.

What is diaphragmatic flutter?

The diaphragm is the main breathing muscle. In diaphragmatic flutter (diaphragm flutter), rapid involuntary contractions of this muscle occur. These can resemble the movements of a belly dancer – hence the name belly dancer’s dyskinesia.

The condition was first described in 1723 by the Dutch scientist Antonie van Leeuwenhoek, who suffered from it himself.

Diaphragm flutter or hyperventilation?Diafragma-flutter of hyperventilatie?

Shortness of breath, chest tightness, palpitations and fast breathing are typical of hyperventilation – but they are also common in diaphragmatic flutter. Because hyperventilation is common and diaphragm flutter is rare, people with diaphragmatic flutter are regularly told for years that they are hyperventilating or anxious.

The key clue is a rapid, rhythmic pulsation of the upper abdomen with its own rhythm, which can be confirmed with fluoroscopy, ultrasound or EMG of the diaphragm.

At a glanceIn het kort

  • Diaphragmatic flutter: involuntary contractions of the diaphragm muscle, visible as abdominal pulsations.
  • Hyperventilation: breathing more than the body needs, causing low CO₂ with tingling and dizziness.
  • Both: breathlessness, chest discomfort and anxiety – and they can occur together.

Frequently asked questions about diaphragmatic flutterVeelgestelde vragen over diafragma-flutter

What is diaphragmatic flutter?Wat is diafragma-flutter?

Diaphragmatic flutter is a rare movement disorder in which the diaphragm contracts involuntarily and rhythmically, usually 0.5 to 8 times per second or faster. It is also called diaphragm flutter, diaphragmatic myoclonus, belly dancer’s dyskinesia or Leeuwenhoek’s disease.

What are the symptoms of diaphragm flutter?Wat zijn de symptomen van diafragma-flutter?

Typical symptoms are visible or palpable pulsations of the upper abdomen, shortness of breath, chest or abdominal pain, a palpitation-like feeling, fatigue, and sometimes hiccups, belching or reflux.

Is diaphragmatic flutter the same as hyperventilation? Is diafragma-flutter hetzelfde als hyperventilatie?

No. Diaphragmatic flutter is an involuntary contraction of the diaphragm muscle; hyperventilation is breathing more than the body needs. The symptoms overlap, so diaphragm flutter is often mistaken for hyperventilation.

Nee.

How is diaphragmatic flutter diagnosed?Hoe wordt diafragma-flutter vastgesteld?

Diagnosis is based on observing the rhythmic abdominal movements and confirming them with fluoroscopy, ultrasound or electromyography (EMG) of the diaphragm. High-resolution oesophageal manometry has recently been described as helpful.

Can diaphragmatic flutter be treated? Is diafragma-flutter te behandelen?

There is no standard treatment. Options described in the literature include medication such as clonazepam, baclofen or carbamazepine, biofeedback, non-invasive ventilation and, in persistent cases, procedures on the phrenic nerve. Treatment is always decided with a specialist.
